As a parent and/or guardian, I do release and forever discharge and hold harmless Calvary Baptist Church and the directors/workers/volunteers thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damages and expenses, of any nature whatsoever which may be incurred by the child participant that occur while said child is a participant in Kids Night Out. Further, I assume all risk of personal injury, sickness, death, damage and expense as a result of participation in this event. Additionally, I understand that photographs may be taken of this event for Calvary Baptist Church purposes only regarding advertising and announcements. Further, authorization and permission are given to Calvary Baptist Church and the undersigned further agrees to hold harmless and indemnify Calvary Baptist Church, its directors, staff and volunteers, for any liability sustained as the result of negligent, willful or intentional acts of said participant(s) including expenses incurred.
